The 2026 midterm elections will shape the healthcare policy environment for the remainder of the Trump administration. Healthcare leaders need to understand which priorities could advance, which could stall, and how the results may affect strategic decisions for years to come.

A Republican trifecta in 2024 produced consequential changes to Medicaid and the ACA through the One Big Beautiful Bill Act (H.R. 1, 2025). The midterm results may change the legislative capacity of Congress, but they are unlikely to reverse that policy direction. Instead, lawmakers will oversee implementation and confront growing concerns about affordability, coverage, and the effects of these changes.
Healthcare leaders must also prepare for policy action on costs, Medicare Advantage, consolidation and competition, Medicare trust fund solvency, and the regulation of AI. The election will help determine which of these issues gain momentum — and where healthcare organizations may face greater financial, regulatory, or strategic pressure.
